Welcome to the Transition Care Program (TCP) Forum.
The TCP Forum is an online communication platform designed to facilitate discussion among the Victorian TCP managers and coordinators with regards to program matters. It also provides a central repository of resources and information related to the TCP, developed by the Department of Health or other sources, such as the Department of Health and Ageing.
