The Transition Care Program (TCP) is a jointly funded initiative between the Australian and State/Territory Governments, which has highlighted the commitment to positive and healthy ageing, with the provision of efficient and effective services that are conducive to the needs of the older people and their carers.
The TCP is available to older people who have completed their hospital episode of care and require more time and support in a non hospital environment to complete their restorative process, optimise their functional capacity and finalise and access their longer term care arrangements.
The program commenced in Victoria in late June 2006, has a total allocation of 1,000 places that are operational across the state, with 19 major health services (metropolitan and rural/regional) recognised as authorised service providers for the program.
